This project focused on mitigating SDOH and promoting health equity throughout the state of New Hampshire. This is going to be achieved by identifying research gaps and using inclusive data collection and evaluation methods. The assessment will measure access to opportunity, health status and outcomes, social connectedness, and community factors. Policies, infrastructure, and resources will be investigated to determine how best to address educational, health and social inequities across the state.
