The goal of this project is to support community partners’ focus on areas that were identified in the Community Health Assessment which often affect those who need more resources and interventions the most. Providing community organization of Health Care partners will improve strategy implementation as well as to monitor effectiveness of strategies in order to provide the most effective supportive and preventative programs to all as well as to identify populations that may need more focus and attention.

This project improves health equity by assisting underserved communities of the Lakes Region of New Hampshire by providing community organization of Health Care partners to improve strategy implementation and monitor the effectiveness of the strategies to provide the most effective support and preventative programs to all. As well as to identify populations that may need more focus and attention.
