If your patients have uncontrollable diabetes, please tell them about the WIC 2.0 study.

Inclusion Criteria

• Female patients >18 years old
• Self-identifies as Black/African American and/or Hispanic/Latina
• Diagnosed with Type 2 Diabetes and Hemoglobin A1c levels >8.0
• Treatment of diabetes with diet, oral hypoglycemic, or insulin
• English fluency
• Stable/permanent housing and/or not living in a shelter program

Exclusion Criteria

• History of diabetic ketoacidosis
• History of acute coronary event
• Dietary medical conditions (Crohn’s disease, ulcerative colitis, end-stage renal disease)
• Medical or serious psychiatric illness (dementia, psychiatric hospitalization, suicidality within the past 6 months)
