If your school or district\u2019s REGISTRATION FOR THE HANDLING, STORAGE AND DELEGATION OF MEDICATION ADMINISTRATION IN PUBLIC AND NON-PUBLIC DAY SCHOOL PROGRAMS and\/or TRAINING FOR UNLICENSED SCHOOL PERSONNEL TO ADMINISTER EPINEPHRINE expires in\u00a0<\/span>June 2018<\/em>, please note the following:<\/strong><\/p>\n

Please \u00a0mail or fax\u00a0<\/span>(617-624-6062<\/a>) a written request for an application to Janet Burke by\u00a0<\/span>April 13, 2018<\/span><\/span>. \u00a0\u00a0Completed applications should be mailed to Janet Burke,\u00a0<\/span>250 Washington Street<\/a>, 5th<\/sup>\u00a0<\/span>floor, Boston, MA 02108 by\u00a0<\/span>May 11, 2018<\/span><\/span>\u00a0<\/span>in order for the registration to become effective after\u00a0<\/span>June 30, 2018<\/span><\/span>.\u00a0\u00a0 Please be sure your certificates of attendance at BOTH the online\u00a0<\/span>Medication Administration in a School Setting: School Nursing Practice in Massachusetts<\/a>\u00a0<\/span>as well as the live program for School Nurse Managers: \u00a0Medication Administration in Schools: What School Nurse Managers Need to Know<\/a>.\u00a0\u00a0<\/span>Please also remember to include a copy of your active RN license with your completed application.<\/p>\n
