Interested youth in this study will need to complete a series of forms to participate, including but not limited to:

1. Interest form
2. Consent/assent form
3. W9 form (see below)
4. Student Information Survey

Youth participants in the research study have the option to be paid by gift card or by check. If you are being paid by check, you will need to complete a W9 form.
