This increasingly recognized epidemic of kidney disease, referred to by some researchers as Mesoamerican Nephropathy (MeN) or CKD of non-traditional etiology (CKDnt), is considered a <a href=\"https:\/\/www3.paho.org\/hq\/index.php?option=com_content&amp;view=article&amp;id=9062:2013-kidney-disease-agricultural-communities-central-america-serious-public-health-problem&amp;Itemid=1926&amp;lang=en\" target=\"_blank\" rel=\"noopener noreferrer\">major public health threat<\/a> by the Pan American Health Organization (PAHO), and is declared a priority by the Commission of Ministries of Health of Central America (COMISCA) in their <a href=\"https:\/\/www.paho.org\/hq\/dmdocuments\/2013\/CD52-8-e.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">52nd DIRECTING COUNCIL Concept Paper<\/a>.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The population most affected by CKDnt differs from that of the global North. In the U.S. and Europe, the major risk factors for traditional CKD are advancing age, diabetes and hypertension. In contrast, CKDnt\u00a0 disproportionately affects younger men, most with no history of diabetes or hypertension. CKDnt epidemics with strikingly similar characteristics are also seen among marginalized populations in other tropical regions, including well-documented epidemics in <a href=\"https:\/\/www.kireports.org\/article\/S2468-0249(19)30127-5\/fulltext\" target=\"_blank\" rel=\"noopener noreferrer\">Sri Lanka<\/a> and <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5969474\/\" target=\"_blank\" rel=\"noopener noreferrer\">India<\/a> as well as reports in <a href=\"https:\/\/www.thelancet.com\/article\/S0140-6736(20)30045-3\/fulltext\" target=\"_blank\" rel=\"noopener noreferrer\">several other locations<\/a>. Agricultural workers appear to be among the groups most affected by CKDnt, particularly those whose communities experience intergenerational poverty, but manual laborers in other industries, such as brick manufacturing and underground mining, also have high rates of disease. Hypotheses regarding the cause of the disease include heat stress and subsequent dehydration, agrichemicals, metals, infectious diseases, medications, diet, low birthweight, and genetic susceptibility, among others. Despite more than a decade of research, the causes remain unclear, owing in part to the likely multifactorial nature of the disease and\u2014until recently\u2014the limited funding available for research.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Our team has been investigating the CKDnt epidemic since 2009 in Nicaragua, and we expanded our work to El Salvador in 2018. The research is informed by a lifecourse approach, which recognizes that environmental, biologic, socioeconomic, and historical factors play a role in an individual\u2019s health throughout their lifetime. In our research, we focus on exposures starting in <\/span><a href=\"https:\/\/sites.bu.edu\/buckdntresearch\/our-research\/early-exposure\/\"><span style=\"font-weight: 400;\">early life<\/span><\/a><span style=\"font-weight: 400;\"> into adulthood. We also consider<\/span><span style=\"font-weight: 400;\"> <a href=\"https:\/\/sites.bu.edu\/buckdntresearch\/our-research\/genetics\/\">genetic susceptibility<\/a><\/span><span style=\"font-weight: 400;\">, which might potentiate an individual\u2019s environmental and <\/span><a href=\"https:\/\/sites.bu.edu\/buckdntresearch\/our-research\/adult-occupation\/\"><span style=\"font-weight: 400;\">occupational exposures<\/span><\/a><span style=\"font-weight: 400;\"> to make one more likely to develop CKDnt.
