Case Management Program
New York State Case Management Certificate Program
This five-course certificate provides participants with an understanding of the core functions and responsibilities of care management and care coordination, an introduction to care transitions, and a review of the resources available to support care in the community. Participants are expected to complete all 5 courses within five months of enrollment. As part of this program, participants will be expected to complete:
- Core Issues in Aging & Disability (6 hours, 6 CEs)
- A Guide to Aging and Disability Networks (4 hours, 4 CEs)
- Assessment with Older Adults and Persons with Disabilities (4 hours, 4 CEs)
- Care Management Practice (2 hours, 2 CEs)
- Care Transitions (4 hours, 4 CEs)

New York State Case Management Certificate Program – EXAM ONLY
The New York State Case Management Certification Program exam only option is offered in collaboration with and funded through the New York Office for the Aging (NYSOFA), and coordinated by the Association on Aging in NY. The program is delivered in partnership with Boston University’s Center for Aging & Disability Education & Research to offer a competency-based certificate program for professional development. The New York State Case Management Certification Program– exam only option will provide participants with the opportunity to challenge their knowledge in case management issues.
