Re-admissions Reduction Project

Boston Medical Center is working towards reducing hospital-wide readmissions. Carrie Solomon, Senior Manager of Strategy Implementation, and Dr. Yurkovic worked with BMC’s data analytics team to identify 14 key factors to make up a risk score for each patient who is admitted to the adult medical inpatient unit. Providers will be able to identify the patients and focus interventions on those who are at the highest risk of 30-day readmissions. The hospital is also interested in ensuring that each patient will continue to receive the interventions post-discharge. Overall, using a risk score to decrease readmissions can improve outcomes while cutting costs, and help avoid penalties to hospitals and payers.
